Early team member is completely MIA, how to proceed?

Sadly, one of our (very talented) UX designers has not been performing and completely unresponsive for the past 2 months. Not responsive to emails, slack or phone calls. team member has common stock with upcoming cliff date is end of March.
What should we do to do the the things the right way?
Notice of termination by email, mail? What do to do about issued stock options?
UPDATE: Spouse confirmed that he is fine
